You may choose from two Dental PPO plans. In both plans, you can see any dentist for your dental care needs.
Basic Dental Plan:
In this plan, your deductible is $75 per person, with a maximum of three people per family required to meet their deductible. The annual benefit maximum is $1,000 and orthodontia for adults and children is covered at 50% up to a $500 lifetime maximum.
Enhanced Dental Plan:
In this plan, your deductible is $25 per person, with a maximum of three people per family required to meet their deductible. The annual benefit maximum is $2,000 and orthodontics for adults and children is covered at 50% up to a $2,000 lifetime maximum.
